DS-2019 renewal time is coming up. It's been hell trying to track down the "Statement of Need" form that us J1-holders need, so i thought i'd linkify it here to help other people out.
The resource itself is buried deep within the DOH website. I was only able to find it after trying different combinations of google terms. The combo that finally worked for me is "exchange visitor doh philippines" (not in quotes).
